F.O.R.M. Curriculum Terms of Use

The F.O.R.M. Curriculum Agreement

This agreement is entered into between you (“Client”) and Glory to Glory Fitness, LLC (“G2G”) effective as the date you accept these terms and conditions by purchasing and accessing The F.O.R.M. Curriculum (the “Program”) in accordance with the following terms.


Your use of the F.O.R.M curriculum or other products and services within the Program constitutes your consent to this Agreement. By accessing the Platform, using our Services, completing a sales order invoice or clicking the check box before the "I have read and agree with the Terms of Use you consent to the terms of this agreement.


Section 1: Scope of Content.

Program content includes teaching on topics related to exercise, nutrition, sleep, recovery and healthy living. G2G will provide Program to Client to facilitate to their students. Program shall commence when payment is made and access is gained to the curriculum. Program is outlined below:

The initial Program duration, participants and termination date shall be specified in the quote or invoice sent. Pricing is detailed on the invoice or pricing sheet. Additional licenses or duration to be mutually agreed upon between G2G and Client. See Exhibit A for Program outline.

Section 2: Client Responsibilities.

(a) Client is solely responsible for the decision to implement and implementation of any ideas, concepts, teachings, or the like, presented in our Program. Client agrees that G2G is not and will not be liable or responsible for any action or inaction or for any direct or indirect result of Client’s use of the ideas, concepts, teachings, or the like, presented in the Program or for Client’s use of any materials provided by G2G in connection with the Program. Client agrees and understands that G2G makes no guarantees regarding the success of Client resulting from the Program. (b) Client acknowledges that G2G reserves the right to terminate a Client’s participation in the Program for violation of the terms of this Agreement.

Section 3. Accident Waiver and Release of Liability.

Client hereby assumes all of the risks of you, your employees, contractors, students, family members or anyone else participating in any/all activities associated with, G2G and its owners, managers, employees, representatives or agents (hereinafter collectively, “G2G”).

In consideration of Client's application and permitting them to participate in the activities contemplated herein, Client hereby takes action for themselves, their executors, administrators, heirs, next of kin, successors, and assigns as follows:

  • (a) Release of liability. Client waives, releases, and discharges from any and all liability, including but not limited to, liability arising from the negligence or fault of G2G, for their death, disability, personal injury, or actions of any kind which may hereafter occur to them including their traveling to and from this activity, including, but not limited to, G2G’s owners and/or their directors, officers, employees, volunteers, representatives, and agents.

  • (b) Indemnification. Client agrees to Indemnify, hold harmless, and promise not to sue G2G from any and all liabilities or claims made as a result of participation in any and all activities contemplated herein, whether caused by the negligence of G2G or otherwise. Client further acknowledges that G2G is NOT responsible for the errors, omissions, acts, or failures to act of its employees, agents, client(s), third-party vendors, or any party or entity engaging in both contemplated activities and unforeseeable actions or omissions.

  • (c) Assumption of risk. Client understands and acknowledges that the training, programs and events held by G2G may expose their participants to many inherent risks, including accidents, injury, illness, or even death. Client assumes all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by them. Client acknowledges that the activities contemplated herein may involve a test of a person's physical and mental limits and carries with it the potential for death, serious injury, and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, equipment malfunction, use of equipment whether proper or improper, premises traffic, lack of hydration, and actions of other people including, but not limited to, customers, participants, volunteers, monitors, third-party vendors, and/or G2G's employees or agents. These risks are not only inherent to participants in the activities but are also present for volunteers.

  • (e) Acknowledgment of agreement. After having read this waiver and knowing these facts, and in consideration of acceptance of client's participation and G2G furnishing services to them, Client agrees, for themself their employees, contractors, students, family members, and anyone entitled to act on their behalf, to HOLD HARMLESS, WAIVE AND RELEASE G2G, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of their participation in the G2G, Program and/or events during onsite training sessions, during any online training sessions where any advice and coaching is given, within any courses or content given, during any off site training sessions including but not limited to in school movement sessions, park sessions, and any facilities you may train at. This agreement and release of liability shall be in effect for this current Program and any and all other programs, courses, or partnership with G2G indefinitely in the future as well as any implementation of our frameworks after the program is over.

  • (f) Medical Disclaimer. The information and materials posted on or accessible through Program are for informational purposes only and are not intended to be a substitute for professional medical advice, diagnosis, or treatment. The Company does not recommend or endorse any specific products, procedure, opinions, or other information that may be mentioned on the Site. Reliance on any information provided by G2G, its affiliated companies, contributors to, or other users of the Site is solely at your own risk.

    Participants should consult your physician or other health care practitioner before starting any exercise program. This is particularly true if you or your family have a history of high blood pressure or heart disease, or if you have ever experienced discomfort while exercising. Never disregard professional medical advice or delay in seeking it because of something you have read in the Program.

This Accident Waiver and Release of Liability form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

Section 4. Miscellaneous.

  • (a) Governing Law and Remedies. Any disputes arising under this Agreement or Client’s participation in the Program shall be governed by the laws of the state of Tennessee without regarding to its principles of conflicts of law. Any dispute arising under this Agreement or Client’s participation in the Program shall be submitted to a state or federal court located in Williamson County, Tennessee, which shall have exclusive jurisdiction over the dispute and to which jurisdiction the Parties irrevocably submit.

  • (b) Class Action Waiver. Each Party waives the right to litigate in court any claim or dispute as a class action, either as a member of a class or as a representative. Any court action hereunder shall proceed solely on an individual basis.

  • (c) No Waiver. No waiver by either party of any default will be deemed as a waiver of any prior or subsequent default of the same or of another provision of this Agreement.

  • (d) Amendments. This Agreement will not be modified or amended except in writing signed by both parties.

  • (e) Entire Agreement. This Agreement contains the entire understanding between the Parties with respect to the subject matter of this Agreement and supersedes any and all prior agreements or understanding between the parties on the subject matter hereof.

  • (f) Severability. The invalidity or unenforceability of any particular provision of this Agreement will not affect the validity or enforceability of the other provisions, and this Agreement will be construed in all respects as if the invalid or unenforceable provision were omitted; provided, however, that if the omitting of such provision results in a material alteration of this Agreement, the remaining provisions of this Agreement shall be adjusted equitably so that no Party benefits disproportionately.

Section 5: Pricing.

  • Client agrees to pay G2G the full amount specified in any purchase order or invoice for a license to Program which will be automatically charged in advance to the account information on file by G2G on the payment(s) due date(s)

  • G2G will keep a record of licensing and duration.

  • G2G is authorized to collect the full amount due by charging any/all cards on file.

    • Rates expire at the end of the 1 year engagement of this agreement.

  • Client represents and warrants that (1) the billing information that Client supplies to G2G is true, correct, and complete, (2) Client is duly authorized to use such accounts, cards, or funds for the purchase, and (3) charges incurred by Client will be honored by its credit card company, bank and other account providers

  • Payments are final and non-refundable. G2G reserves the right to extend grace to the client for extenuating circumstances regarding payment, this will be at the discretion of G2G. If Client does not comply with its payment obligations, G2G may revoke access to Program.

Exhibit A: Program is detailed below:

Program includes 32 modules covering:

  • Functional Fitness: (8 lessons)- Learning how to move well over a lifetime.

  • Optimal Nourishment: (8 lessons)- Learning how to enjoy food, while also fueling our body well.

  • Rest and Recovery: (8 lessons)- Learning how to stay refreshed in our mind and body.

  • Multiplied Maintenance: (8 Lessons)- Learning the foundations of thinking for sustainable growth in healthy living, while helping others do the same.

  • Fuel Challenges: Students will be challenged to make some of their own snacks and meals.


Each module contains:

  1. An 8-12 minute tailored teaching (Animated video)

  2. Warm-up video

  3. Functional Fitness video

  4. Cooldown video

  5. A pre-created assessment


Client will have full access to Program to facilitate it to participants. G2G will support the Client's facilitator with any needs they may have at G2G's discretion.
